When the Owner/Builder or Contractor is not available to complete the permit application at Cith Hall, it may be completed and signed by the Owner/Builder or Contractor and submitted by proxy. The application declarations must be completely filled out and signed in the appropraite places.

The carrier of the permit application must provide a legible copy of:

• The contractors driver's license or other approved identification; and
• A copy of the front and back of the signed contractor's state license card; and, if applicable
• A copy of the incorporation papers indicating the authorized person(s) and their proof of identification.

The signatures on the licenses and permits applications will be expected to match. A letter, noterized letter or other non-official authorization is not an acceptable form of authorization.
